The name of this Plan is the Plumbers, Pipe Fitters and Mechanical Equipment Service Local Union No. 392 Health and Welfare Plan. The name of the Trust Fund through which this Plan is provided is the Plumbers, Pipe Fitters & Mechanical Equipment Service Local Union No. 392 Health and Welfare Fund.

Source of Contributions, Collective Bargaining Agreement - The Fund receives contributions from employers who have entered into collective bargaining agreements with the Union. Contributions are made monthly to the Fund and enable employees working under such agreements to participant in the Plan.

Employees are entitled to participate in this Plan if they work under one of the collective bargaining agreements and if their employers are required to make contributions to the Fund on their behalf.

Other persons who may be eligible for benefits under this Plan are full-time employees of Union, Health and Welfare Trust Fund and Credit Union, and persons who are full-time employees of the Greater Cincinnati Building and Construction Trades Council and/or full-time employees of the Ohio State Association of the United Association of Journeyman and Apprentices of the Plumbing and Pipe Fitting Industry, AFL-CIO, provided such employee is also a member of the Union and resides in the geographical jurisdiction of the Union. In addition, part-time employees of the Union or part-time employees of the Health and Welfare Trust Fund may be eligible for benefits under this Plan provided any such employee accrues sufficient hours of eligibility.

The Fund also receives self-payments from employees, retirees, and dependents for the purpose of continuing coverage under the Plan according to the self-payment eligibility rules of the Plan.

Accumulation of Assets: Payment of Benefits – Employer contributions are received and held in trust by the Board of Trustees pending the payment of benefits and administrative expenses. Income is also realized from invested assets. Death, Accidental Death and Dismemberment, hospital, medical, surgical, disability and Medicare supplement benefits are provided by the Fund on a self-funded basis. When benefits are self-funded, benefits are paid directly from the Fund to the employee or retiree (unless benefits are assigned).

Plan Year – The Fund’s financial records are maintained on a 12-month calendar year basis. The end of the Plan year is December 31.
